We provide free evaluation, and tailor the care to the needs of the individual, and his or her family. Our care providers include Caregivers, Certified Nurse Assistants, Certified Home Health Aides, Registered and Vocational Nurses, Physical, Occupational and Speech Therapists, and Licensed Dieticians. All are experienced at home nursing, compassionate, and attentive. All of our care professionals are thoroughly tested and screened. Physical Therapy

Our Physical Therapists (PT) work with the patients and their physicians to develop an individualized plan of care to help our patients achieve independent and optimal functional mobility. Common diagnosis and procedures for Physical Therapy services at home are Total Hip Replacement surgery, Total Knee Replacement surgery, Osteoporosis, Fractures, Amputation, Acute Back Pain, Head Injury, Stroke, Degenerative Joint Disease, Osteoarthritis, Rheumatoid Arthritis, Osteomyelitis, Herniated Disc.

Physical Therapy Services includes:

  • Assessment and evaluation
  • Recommendation and Use of Assistive Devices, Prosthetics, and Orthotics
  • Gait, Transfer, and Other Mobility Skills Instruction

Occupational Therapy

Our Occupational Therapists (OT) work with the patient’s physician to develop and carry out an individualized treatment plan for each patient to help recover from or adapt to disability and/or illness.

Occupational Therapy Services include:

  • Assessment and evaluation of patients’ status
  • Planning and implementation of therapeutic programs to:
  • Restore physical function
  • Promote strength, coordination and balance
  • Restore sensory /integrative functions
  • Improve level of independence in the safe performance of the activities of daily living (ADL)
  • Teach compensation for impairments

Speech Therapy

Our Speech Therapists work with your physician to develop individualized plan of care designed to teach speaking, reading, or writing; improve auditory comprehension; improve orientation, memory and judgment skills; strengthen oral muscles for speech and swallowing; teach esophageal speech or use of electro-larynx; or develop alternative communication systems. Speech Therapy is commonly recommended for patients with CVA, Parkinson’s, ALS, MS and other neurological diagnosis, post-surgery patients, Pneumonia/Septic patients and Dementia patients.

Medical Social Services

Medical social services greatly benefit patients, family members and caregivers who are currently experiencing difficult life crises. Our licensed and experienced medical social workers assess patient’s psycho/social situations in order to assist and educate patient and family members about available community resources.

Medical Social Services include:

  • Helping patients recognize the need to seek help
  • Assisting patients and their families in resolving various health related issues
  • Providing emotional support
  • Helping families acquire access to community resources
  • Counseling and health education

Home Health Aide

Under the supervision of our Registered Nurses (RN), our Certified Home Health Aides (CHHA) compassionately provides the following services:

  • Medication reminders
  • Light housekeeping and laundry
  • Providing assistance with bathing, dressing and grooming
  • Providing assistance with ambulation
  • Meal preparation and providing assistance with eating
  • Incontinent care
  • Companionship

All of our Certified Home Health Aides are our own employees whom are trained, and continuously evaluated for competency, and thoroughly background checked, insured, and bonded.
